hexagon-lldb not hitting breakpoints

I'm able to run the simulator.  And I'm able to connect from hexagon-lldb to hexagon-sim by gdb-remote.  But the breakpoints I set are not triggered.  Is there some option or setup I'm supposed to set besides the port # when connecting to the simulator from the debugger?

I'm using Hexagon SDK 3.4.3 tool version 8.3.02.  But I get the same issue with SDK version 3.5.2, whether it's application under development or one of the SDK examples.
